Problem Description: When trying to format an Elite 1.3GB disk you will get
			an Assertion Failure. This causes the failure to
			to write a label to the disk, so subsequent attempts
			to use format will show drive type as unknown

Install Instructions:

As root, make a backup copy of format:

	mv /usr/etc/format /usr/etc/format.FCS
	
Now install the patched format:

	cp format /usr/etc/format
